Submit Your Completed Application Materials

Completed application materials include your:

  • Application: Contains the basic data and demographic information, and statistics from your center
  • Self-Evaluation Report (SER): In the SER, you describe how your birth center meets each part of the Standards. The SER is the most important part of the accreditation process.  It includes supporting documents as indicated in the Accreditation Manual instructions.
  • Copy of your Policies and Procedures Manual
  • Application fee (See a list of all accreditation fees.)

Your application is completed online and must be submitted 2 months before your site visit. The SER, all supporting documents and your Policy & Procedure manual are submitted electronically by uploading files to the CABC project managemnt system. They must be submitted 1 month before your site visit to allow time for the site visit team to conduct a preliminary review of the materials. This allows the site visitors to notify the center if there are any documents missing from your application.

Document Submission

ALL documents must be submitted digitally.

  1. Contact the CABC office to request a username and password to the CABC project management system (BaseCamp)
  2. Use the link in the invitation e-mail to create a user name and password.
  3. Log-in to BaseCamp using the user name and password that you create.
  4. Click the Messages tab.
  5. Post a new message with the title “Accreditation Materials.”
    1. Attach all your files to that message. If an individual file is over 100MB, compress it to a zip file.
    2. Be sure to check the box below the message area to send your materials to the CABC Administrator.
  6. You may upload your documents as they are completed. Be sure to follow the above steps for each upload.
  7. Send your payment by check to the address above using certified mail.
